Medical model adult day health care center serving the elderly
A medical model , multi -ethnic adult day care program serving the brooklyn community for elderly people who have multiple medical issues and/or are physically disabled. Services provided are medical/nursing monitoring and case managment, medication management, social work services, nutrition, therapeutic recreation/trips, rehabilitation, transportation, hot meals and snacks. Clients live at home and attend this program for the above services. RNs and CNAs on staff. Languages spoken are English, French, Creole, Spanish, Russian, Chinese. Open Mon-Fri.
